Working as a member of a multidisciplinary health care team maintaining close liaison with Community Nursing, Allied Health Services and Home Care South. Inthis role you will be providing support and care to Home Care South clients in their own homes.Function effectively as a home care support worker under the Tasmanian Home and Community Care (HACC) guidelines, and the Commonwealth Home Support Programme (CHSP) guidelines. 



Key Accountabilities and Responsibilities:

The Home Care Support Worker, under the general supervision and direction from the Manager of Home Care South, is responsible for:

  • Providing a high standard of home and personal care services, including maintaining a high standard of hygiene and cleanliness in practice and personally.

  • Providing timely feedback to the Manager of Home Care South on significant changes to a client’s health and circumstances

  • Maintaining client confidentially.

  • Promoting a positive image of the service to the public, including the displaying of approved means of identification, and wearing a well-maintained supplied uniform.

  • Promoting wellness and reablement within Home Care South’s service delivery.

  • Champion a child safe culture that upholds the National Principles for Child Safe Organisations. The Department is committed to the safety, wellbeing, and empowerment of all children and young people, and expect all employees to actively participate in and contribute to our rights-based approach to care, including meeting all mandatory reporting obligations.

  • Where applicable, exercise delegations in accordance with a range of Acts, Regulations, Awards, administrative authorities and functional arrangements as mandated by Statutory office holders including the Secretary and Head of State Service. The relevant Unit Manager can provide details to the occupant of delegations applicable to this position.

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Current Driver's Licence

Applicants should note the following criteria are desirable:

  • Current First Aid Certificate

  • Certificate lll in Individual Support or other relevant qualification

Applicants should note, the position features:

  • Travel to clients' homes using your own registered and insured transport
